The system will sign the user ff and the Sign On screen will display. 6) T quickly sign ff frm anywhere in the Pint f Care sftware, select Hme n the keybard. The system will sign the user ff and the Sign On screen will display. NOTE: if yu are stepping away temprarily (few secnds t 10 minutes), yu may use the Key icn which appears in the tp left hand crner f mst CPSI screens t lck yur sessin. This actin will nt cmpletely sign yu ff the system, but will hide any data n yur screen and minimize yur screen. When yu return just maximize yur minimized screen and re-enter yur passwrd and yu will be taken t where yu left ff. This secures yur patient s PHI while yu are mmentarily away. Chapter 4 Pint f Care Whitebard: 4.1 Overview This chapter will discuss different aspect f the Pint f Care Whitebard. 4.2 Hw t Access Pint f Care Whitebard T access the Pint f Care Whitebard, sign n and frm the Hspital Based Menu select POC access. 4.3 White bard access tabs: Review Vital Signs-Allws users t review Vital Sign entries fr selected patients. Patient List-The Patient List will allw the user t search fr a patient using several different indexes. Selecting a current patient is the first step. Yur nursing statin, any nursing statin, patient name, patient rm number, and patient accunt number are available t select patients.Print Reprts-Selecting Print Reprts ptin allws user access t Base Menu Reprts. Change Default Printer-Selecting Change Default printer ptin allws user t set their wrkstatin printer, search by number r chse frm a list f netwrk printers. There is als an ptin t remve default printer. 4.4 Patient Whitebard Patient selectin within the Pint f Care applicatin will allw the user t view patients specific t them r patients listed within the nursing department. The patient selectin will allw a user t select the patient s fr whm they are administering care. Once the user has selected their patients, they will display fr the user each time the user signs n t Pint f Care and selects the Patient Selectin ptin. This will turn t a - when selected 4) T remve a patient frm My Patients select n the red - sign t the right f the Patient s name. 5) T view the patient listing fr the department, select the Census tab at the tp f the menu. 6) All patients listed n the department will display the fllwing clumns: Rm Number: If yu prefer t srt patient s by their rm number, click n the wrd ROOM in the header until it turns red. Patient Name: Yu may change the srt t srting by the patient s last name alphabetically by clicking n the wrk NAME in the header, until it turns red. Accunt Number: a unique identifier fr this visit nly. STATS The Whitebard shws stats represented by fur symbls r icns. The Test Tube indicates new results are available. The Exclamatin Mark indicates critical results are available n the patient. The Chart Symbl (lks like a dmin r Rubic s cube): indicates there are results ther than labs available fr review. (CT Scans, X-ray, scanned images etc). The red and blue Capsule icn indicates that a new r changed medicatin has been added t the patient s medicatin prfile and Medical Recrd Number Physician: indicates the primary physician n recrd. Lcatin- will allw users t set where the patient is actually lcated within the facility fr tracking purpses. Once the patient is selected, a red - displays 7) Frm the Census r My Patients tab. Select the patient in which t perfrm dcumentatin n by selecting that patient s name. The system will then display the Patient s Functin menu. 4.5 Other Functins Tab 1) Mail Functins-Mail is used as a cmmunicatin tl and can be sent between users, t any department. The user can als view new mail r ld mail. NOTE: We are in the prcess f develping the mail functins at WHMC and it may nt be fully peratinal in all departments. Fr areas in which it is functinal: Select Mail Functins frm the Pint f Care Whitebard Screen. Chse Send Mail t Any Department, Send Mail t Any Emplyee, r Send Mail t Any Physician s Clinic. (sme functins will nt be used in its entirety) Select the first few letters f the department, user s last name, r physician s clinic. T send mail t a department, select Send Mail t a Department. Hld dwn the cntrl key t select mre than ne dept Hld dwn the shift key t select a range f departments Select Send t department(s) 2) Change Department-Change Department is a feature that is shwn t the IS Department and ther departments n an individual basis. 3) Physician Privileges List -Physician Privileges can be viewed r printed frm the Pint f Care Main Selectin screen. This feature allws access t the selected physician s diagnsis and prcedure privileges withut displaying cnfidential infrmatin pertaining t the physician. Frm the Pint f Care Main Selectin screen, select Physician Privileges List. Select a physician using ne f the fllwing display ptins: Select Physicians by name: Displays a lkup f physician names alphabetically Select Physicians by number: Displays a lkup f physician numbers in numerically Press Enter t display additinal pages f privileges.Chapter 5 Chart Type Selectin: 5.1 Selecting a Chart Type Each clinical nursing area is assciated with a specific chart type. Selecting a chart type prmpts the system t display questins and answers, which are develped by nursing services. Each chart type relates t a clinical area where a patient is admitted. It is imprtant t have the crrect chart type selected as they may als drive what reprts and data n reprts that can be seed fr yur patient. If the patient is transferred t ICU, the chart type shuld be changed t ICU. The chart type can be selected frm the Patient Functin menu. 1) Select a patient frm the Patient Whitebard by tuching the patient's name. 2) Select exit t acknwledge the Med Act 3) Select the bx labeled, Chart Type 4) Select the apprpriate chart type, it will highlight red. 5) Tuch the arrw in the tp right crner t return t the previus screen. Chapter 6 Virtual Chart: 6.1 Overview Virtual Chart allws access t vital signs, demgraphics, test results and POC applicatins. This chapter will discuss the functinality f Virtual Chart. 6.2 Accessing Virtual Chart The Virtual Chart is nly accessible via the POC Whitebard. Patient Demgraphics Patient Demgraphics including Patient Name, Accunt Number, Age, Sex, Height, Current Weight and Rm Number display acrss the tp f the Virtual Chart.Select Immunizatins t view immunizatins and the date they were received. Select Allergies t return t the allergy listing. Patient Lcatin- Patient Lcatin will allw users t select where the patient is lcated within the facility fr tracking purpses. Lcatins selected n the Virtual Chart will crss ver t the lcatins n the Whitebard. Demgraphics -This sectin will display the mst current data charted. Vital Signs- Bld Pressure, Temperature, Respiratins, Pulse and 02 Saturatin. Only the mst recent vital signs charted will display. Chart Type Each clinical nursing area is assciated with a specific chart type. The chart type can be selected frm the Patient Functin menu. Select a patient frm the Patient Whitebard by tuching the patient's name. Select the bx labeled, Chart Type Select the apprpriate chart type, it will highlight red. Tuch the arrw in the tp right crner t return t the previus screen. Current Date and Time-The date and time can be changed t meet yur needs, t d s, please d the fllwing: Select Current Date and Time. Enter the desired date and time Remember t return t the current date time and time when cmpleted. T select the current date and time chse again the current date and time bx. This bx will have the date and time that yu previusly selected. Chse this date and the wrds Current date and Time will appear. Nte Entry-The Nte Entry ptin allws fr a nte t be left fr a specific physician r fr all physicians. If the nte is left fr a specific physician, the nte will be viewable frm the ChartLink Applicatin (A physician prtal int CPSI). Select Nte Entry. If there is an existing nte r ntes, they will display with the date, time, physician, and status.Optins t attach image include Scanner, PC, Server, r Digital Signature Directry. NOTE: If attaching image via a scanner the system will detect the scanner and prmpt the user thrugh scanning prcess. Select Descriptin t name the Image frm the drp dwn bx. Select the File Name t assciate a Descriptin t the Image. Select the Binculars Icn t select file t attach. Select Save File. NOTE: A pp-up bx t allw dcumentatin f the data the image was attached t may display. Select File List t view attached images Select Back Arrw in upper left crner t exit screen. Late Entry Select Late Entry. Select the Date and Time f the ccurrence, then OK t prceed r Cancel t return t the Virtual Chart. The script in the Late Entry Bx will turn blue and the selected date and time will display in blue in place f Current Date and Time. All infrmatin charted will reflect the changed date and time. T return t the current date and time, select Late Entry again n the Virtual Chart. Transfer-Patients can be transferred frm the Virtual Chart. Please see unit specific guidelines prir t transferring a patient frm ne unit t anther. Select Transfer Cmplete the required fields. Select Transfer t prcess r Cancel t abrt the Transfer. Discharge-Patients can be discharged frm the census frm the Virtual Chart. Please see unit specific guidelines prir t discharging any patients. Select Discharge Cmplete the required fields. Select Discharge t prcess r Cancel t abrt the Discharge. Results- All results may be viewed by selecting n the All buttn. Select New Only t return t the new results list. New results can be identified by their blue fnt. Once results have been reviewed, the fnt clr changes t black. Staff Cmmunicatin -Is a cmmunicatin tl nly. The infrmatin entered in this area des nt save t the patient's chart and it des nt print t any POC Reprts. Select anywhere in the white bx titled Staff Cmmunicatin.MultiDisciplinary Charting-are used primarily by ur ancillary staff, i.e. physical therapy, respiratry therapy, dietary etc. Viewing Clinical Histry All Stays is selected; users will see all POC reprts fr the current accunt and reprts that have been set t mve t Clinical Histry frm previus accunts. Chapter 7 Demgraphics: 7.1 Accessing Demgraphics This infrmatin, entered by the admissin clerk during patient registratin, will autmatically display n the Virtual Chart. The nurse shuld review demgraphic data fr cmpleteness and accuracy. Chapter 8 Electrnic Frms: 8.1 Electrnic Frm Selectin Electrnic frms are used fr thse frms which have a ne time use feature. Hwever we have altered this general feature t enable the use f a care plan fr use by clinical ancillary departments. Hwever, at WHMC the Nursing staff use the Nursing Prblem List which is in the flwcharts sectin fr their care planning needs. Fr the mst part yur E-frms are used ne time, and then cmpleted and saved. Fr frms which require a patient (r their representatives) signature, we als ask that the frm be lcked by the end user nce the last signature required n the frm is btained (see sectin 8.4 fr hw t lck a frm). E-frms may be accessed frm varius pints within the system. The mst cmmn place will be frm the Virtual Chart. Frm the Virtual Chart select the tab Electrnic Frms Frm the drp dwn menu select Electrnic Frms Frms that have already been selected fr the patient will display in the Previusly Selected Electrnic Frms prtin f the screen. The creatin date and created by infrmatin will als shw here. Select Cancel then chse the existing frm fr the patient.Cmplete the frm in its entirety. Yu will encunter different ways t enter the infrmatin. Yu may need t select yur answers by using a drp dwn menu, a free text bx r a buttn. Checkbxes are als available. If a drp dwn menu presents the ptin ther this is an ptin fr yu t free type an answer in. If yu ntice a frm with a questin mark Click n the questin mark. This will bring up additinal infrmatin fr yu t make the apprpriate decisin. See sectin 8.5 which has imprtant infrmatin abut the E-frms Tl Bar Optins. 8.2 Using the Signature Area The Signature Area is used t electrnically sign a frm. The frm can be signed using a Digital Signature Pad. Signature Area with Digital Signature Pad Select the Signature field. Select the Digital Signature Pad icn. Sign the dcument using the Digital Signature Pad. Accept the signature via the Digital Signature Pad r by selecting the Accept Signature 8.3 Charting n Vital Signs Navigatin f the Vital Signs cntrls n an electrnic frm is very similar t the way that vital signs are dcumented and navigated within the Pint f Care flwchart. A lcked frm is viewable in ChartLink. A lcked frm cannt be charted n until it is unlcked. Only the user wh lcked the frm can unlck it. Previusly Selected Frms windw n the Electrnic Frms Selectin Screen will list the frm with the lck status shwing the user's name and date and time the frm was lcked. At WHMC, we ask that yu lck Cnsents after the last signature is btained t avid any edits t the frm.Once a frm has been lcked, nly the user that perfrmed the lck can unlck it. The frm cannt be charted n if it is lcked. Frm must be lcked in rder fr it t be previewed n a Patient in ChartLink. Select the Prev Page Arrw t g t the previus page f the Electrnic Frm. Dcumentatin will be saved n this page. Select the Next Page Arrw t mve t the next page f the Electrnic Frm. Dcumentatin will be saved n this page Select the drp-dwn arrw beside PG t mve t a specific page f the Electrnic Frm. Select the Disk Icn t save the data that was dcumented n the Electrnic Frm and remain in the frm. Select the Print Icn t save and print the Electrnic Frm. Calculate Icn: This is nt currently used in the hspital Electrnic Frms Applicatin. It is used in clinics t calculate level f care in the Ambulatry Care Dcumentatin (ACD) mdule. This buttn will abrt unsaved changes t the current page and exit t the frms menu. Chapter 9 Flw Charts: 9.1 Flw Chart Selectin When Flw Charts are accessed fr the first time fr a new admit, the system will default t the New ptin. The Flw Chart selectin screen will display the individual Flw Charts.Select Flw Chart frm the Flw Chart tab within the Virtual Chart Select the desired Flw Chart, either frm selecting new r existing flw charts. If yu chse a new but enter a flw chart that has already been pen, an alert will infrm yu that, that particular flw chart already exists. Cmpnents f the Flw Chart selectin screen, select frm the fllwing ptins: Camera - T attach pc images t answers in a Flw Chart, select n the camera. (NOT IN USE AT WHMC AT THIS TIME). Ntepad T display additinal dcumentatin fr the Flw Chart, select the Ntepad ptin. Clipbard T display the patient s demgraphics, select the Clipbard. Ten clumns (time frames) display acrss the tp f the flw chart display. There are maximum f 500 (five hundred) clumns that can be created. As dcumentatin surpasses 500 clumns, clumns at the beginning f the flw chart will n lnger display. Dcumentatin will nt be lst and can be viewed via the Patient Prgress Ntes. After 500 entries, yu will need t COMPLETE the flwchart and pen a new ne f the same type t cntinue additinal entries. Hrizntal Arrws The system displays each entry s date and time acrss the tp f the flw chart. The hrizntal arrws are used t display a selected time frame. Vertical Arrws The system displays as much f the Flw Chart n the screen as pssible. The vertical arrws are used t display a certain categry sectin. Exit T return t the Patient Functins screen, select Exit. 9.2 Camera Optins (this sectin is nt currently functinal at WHMC) The camera ptin can be utilized fr attaching pc images t Flwchart answers. Ability t uplad and attach images is based n user security. Only ne image at a time can be attached t a specific answer in the Flwchart. T access this functin, select the Camera icn n the flwchart. Select a Dcument Title frm the Drp-Dwn Bx. Then select Stre. Then select image t stre. When image has been stred the system will return t the Flw Chart in Image Mde.The Ntepad ptin can be utilized fr dcumenting additinal ntes. Dcumented infrmatin can be reviewed frm the ntepad ptin. The infrmatin will nt print t any particular vital sign, assessment r activity categry. All infrmatin charted via the ntepad ptin will print n a separate reprt titled Patient Prgress Ntes Flw Chart Ntes. This reprt will be the last printed page f the Patient Prgress Ntes. Select the Clipbard icn n the flwchart. T review an existing entry, select n the entry. If the entry needs t be amended, select the Amend ptin. Only the persn wh entered the entry can amend the entry. T enter an entry, select the New Page ptin A New Ntepad Entry edit bx will display. Select inside the edit bx t display the cursr. Then type in the infrmatin. Select the Update ptin t save the infrmatin. 9.4 Reviewing Patient Demgraphics Patient demgraphics can be reviewed r changed frm a flw chart. Select the Clipbard icn in the upper right-hand crner f the flw chart. Select the back arrw in the left-hand crner f the screen t return t the flw chart. The mst recent set f vital signs will appear n the frnt f the Virtual chart. Vital signs can be reviewed by selecting a flw chart in which all vital signs entered will appear. Yu may als chse the Vital Signs reprt t see a list f all vitals that have been taken. Within the whitebard, select Review Vital Signs. Select desired patient r patients Recrding Intake Intake and utput are entered within the flw charts. Select the questin New Intake Select the methd f intake by selecting the apprpriate bx t display a numerical keypad. Enter the amunt f intake by selecting the numbers n the numerical keypad.T use the Other ptin, select Other n the Intake menu t display the Other Optin menu. When Other is selected, a bx will appear fr free typing, enter the desired mde. This will appear in the bdy f the flw chart, and will remain there fr further dcumentatin fr the remainder f the stay. Recrding Infant Breast Feeding The ptin fr dcumenting the length and number f times an infant was breast fed can be perfrmed via the Nursery and Pediatric chart-types. Recrding the Number f Times an Infant Breast Fed T recrd the number f times an infant nursed, select Breast Feeding (X) frm the Intake menu t display the Breast Feeding (X) Intake menu. T dcument the quality f the feeding, select the apprpriate bx. T dcument the number f times an infant nursed n either the right r left breast, select R Breast r L Breast, as apprpriate. Enter the number f times the infant nursed n the numeric keypad. Select Update Recrding the Length f Time an Infant Breast Fed T recrd the number f minutes an infant nursed, select Breast Feeding (MIN) frm the Intake menu t display the Breast Feeding (MIN) Intake menu. T dcument the number f minutes an infant nursed n either the right r left breast, select R Breast r L Breast, as apprpriate. Enter the number f minutes the infant nursed n the numeric keypad Recrding Output Intake and utput are entered within the flw charts.
