On-line haemodiafiltration (OL-HDF) is based on the controlled administration of large volumes of replacement ultrapure dialysate to the extracorporeal blood circuit of dialysis. It is currently the most effective technique for eliminating small and large uraemic toxins. Recent studies have observed a direct association between a decrease in mortality and the received convective volume. The latest Fresenius 5008 (5008 CorDiax) monitor software update enables the automation of the replacement volume without requiring total protein and haematocrit values, with the aim of maximising convection. Objective. The study’s objective was to evaluate the recent version of the 5008 monitor software, compared to the previous version, on the impact on total convective volume. Material and method. Each patient was analysed in 6 sessions; 3 with the 5008 monitor and 3 with the 5008 CorDiax monitor. The replacement volume, total convective volume and dialysis parameters were determined in each session. Results. No significant differences were observed in blood, venous or transmembrane pressure, nor was there an increase in the number of alarms or coagulation of lines or dialysers. Conclusion. Unlike conventional hemodialysis treatments, which rely almost solely on diffusion-related mechanisms for solute removal, hemodiafiltration (HDF) allows more efficient removal of higher molecular weight toxins due to convective transport mechanisms. To facilitate the removal of these toxins in HDF treatment modalities, dialyzers with highly efficient high-flux membranes are necessary. This study assessed the large uremic toxin removal ability of a high-flux dialyzer (FX CorDiax 60) specifically designed to facilitate convective therapies compared with a standard high-flux dialyzer (FX 60). Methods. In an open, randomized, cross-over, single-center, controlled, prospective clinical study, 30 adult chronic hemodialysis patients were treated by post-dilution online HDF with the FX 60 or the FX CorDiax 60 dialyzer. All other dialysis parameters were kept constant in both study arms. The reduction rate (RR) of blood urea nitrogen, phosphate, ?2-microglobulin (?2-m), myoglobin, prolactin, ?1-microglobulin, ?1-acid glycoprotein, albumin and total protein as well as the elimination into dialysate was intraindividually compared for the two dialyzer types. We examined the hypothesis that mixed-dilution online hemodiafiltration (MIXED) rather than predilution online hemodiafiltration (PRE) could enable patients with low blood flow rate (Qb) to benefit from advantages of convective therapies. Albumin losses (Alb) were also measured in 12 patients. MIXED was always found to be more efficient compared to PRE notably for middle molecules (MM). However, this technique is supposed to have an additional cost which may be an obstacle to increase its development in hemodialysis centers. The aim of the study is to achieve an accurate pharmaco-economic evaluation for determining the real overcost of online hemodiafiltration (OL-HDF) in comparison with high flux hemodialysis (HF-HD) using standard priming. We have identified the additional costs related to the consumables and monitors and the additional costs imposed by the technique itself (water consumption and microbiological analysis). In the center, more than 28,000 sessions per year are performed with 70 in OL-HDF (90 post-dilution). View Show abstract Cost comparison of online haemodiafiltration with high-flux haemodialysis Article Oct 2011 J NEPHROL Tom Oates Jennifer Cross Andrew Davenport Haemodiafiltration increases convective clearances, and online haemodiafiltration (OL-HDF) was introduced to reduce costs of producing large volumes of ultrapure water. Previous reports have suggested that OL-HDF may be a cost-saving therapy by reducing recombinant human erythropoietin requirements and those for other medications. We therefore costed OL-HDF with high-flux haemodialysis. They were then prospectively followed for 12 months. Depending upon whether blood lines with a cuvette for measuring relative blood volume were used, OL-HDF was either slightly more expensive per session, by ?1.16, as the cost of the reinfusion line outweighed any potential savings in 0.9 saline usage, or cheaper by ?0.78 if standard blood lines were used. Although there were initial increased costs for more frequent testing of dialysis machine water quality. It could be argued that similar water quality is required for high-flux haemodialysis using dialysers with increased internal filtration. Depending upon the choice of blood lines, OL-HDF was either a slightly more expensive or a cheaper treatment per session compared with high-flux haemodialysis in our centre. Treatment with OL-HDF also led to modest cost savings on phosphate binders. Prefilter reinfusion (pre-HDF) may overcome this problem, but plasma dilution may affect the overall efficiency of the technique. In this study, an experimental system of online HDF with combined prefilter and postfilter infusion (mixed HDF) was evaluated and compared with the traditional predilution and postdilution modes. K(DQ) for Phos and beta(2)m were higher on post-HDF in only absolute values. Similar differences were found for instantaneous dialyzer clearances (K(I)) at 60, 120, and 180 minutes of the sessions, with a common trend to decrease with time. K(UF) and the apparent beta(2)m sieving coefficient showed their lowest values toward the end of post-HDF sessions. Increasing filtration fractions (FFs) were associated with increasing transmembrane pressure (TMP) and solute clearances up to FF values of 0.45. These were values achieved in only post-HDF, at which point the curve of the relationship between TMP and FF assumed its steepest exponential trend. Mixed HDF, by better preserving the characteristics of water and solute transport of the membrane, ensured safer operating conditions than post-HDF, while achieving similar removal of small- and large-size solutes. We performed a crossover study following an A1-B-A2 design involving 10 patients. Phase A1 was 1 week of thrice-weekly bicarbonate hemodialysis with the FX dialyzer, 4 h per treatment; phase B was performed with a similar treatment regimen but with a new FX CorDiax dialyzer and finally the phase A2 was repeated with FX dialyzer as the former phase.
